Are you needing information for the 1st day of school? 

Additional information & forms can be found on the Parent Resource Page

LPS Opt-Out Request

Parents,  if you would like for your student to be “OMITTED” from any district directory publication, you may do so with the attached form.  This means that your student’s name or picture would NOT be publicized.  Examples of items they would be omitted from, by name and photo are:

·         Yearbook photo

·         Video broadcast at school

·         Newspaper article

·         Team sports photo’s

·         Fine arts music program listing with their name, etc. 

If you peruse the form, there is a more complete list of examples. 

If you wish to opt out, please fill out the form and turn it into the counseling secretary by October 1st.
